Congenital diaphragmatic hernia (CDH) is a rare congenital anomaly. It is characterized with the structural defect of the diaphragm in the intrauterine period. A male neonate with diagnosis of CDH was hospitalized after vaginal delivery at 39 weeks’ gestation. The case was resuscitated and referred to our Neonatal Intensive Care Unit (NICU) on the second day, diagnosed with Bochdalek hernia and underwent surgery. Chest physiotherapy (CP) was started on postoperative 24th day. CP was applied for a total of 24 sessions, twice a day, four days/week, for three weeks, each 30-40 minutes. CP was started with bilateral vibration to the upper apical lobes in modified drainage positions and passive mobilization of the upper limb. CP, performed by positioning the lung lobes in need, consisted of vibration and gentle percussion for 3-5 minutes with distal finger phalanges. Due to the chest X-ray findings, the posterior or lower basal lobes were studied more intensively. Aspiration was performed after physiotherapy in each session. The sessions were ended by placing the patient in side-lying position or a prone position to increase oxygenation. The patient's vital signs, the type and color of secretion, chest radiography, respiratory sounds, nutritional type, and respiratory pattern were evaluated before, during, and after CP. No complications were observed during CP. It was observed that secretion excretion improved, weaning from the mechanical ventilator accelerated, drainage in the chest tube got easier, oral feeding increased, and vital signs didn’t change. Respiratory sounds and chest X-rays showed that the infiltration and atelectasis areas decreased towards the discharge process.

Facioscapulohumeral Muscular Dystrophy (FSHD) is in the top three list of all dystrophies with an approximate 1:8000 incidence. It is not a life-threatening disease; however, the progression of the disease extends over being wheelchair bound. Despite some drug trials continuing, including DUX4 inhibition, TGF-ß inhibition and resokine which promote healthier muscle, there is not an applicable treatment option for FSHD today. Still, there is a need for new agents to heal, stop or at least slow down muscle wasting. Current FSHD studies involving nutraceuticals as vitamin C, vitamin E, coenzyme Q10, zinc, selenium, and phytochemicals as curcumin or genistein, daidzein flavonoids provide promising treatment strategies. In this review, we present the clinical and molecular nature of FSHD and focus on nutraceuticals and phytochemicals that may alleviate FSHD. In the light of the association of impaired pathophysiological FSHD pathways with nutraceuticals and phytochemicals according to the literature, we present both studied and novel approaches that can contribute to FSHD treatment.

Sargassurn vulgare was sampled by free dives in Iskenderun Bay, Hatay, Turkey, in September 2018. Sargassum vulgare is a material with high economic value because it has compounds that can be used in medical applications such as alginic acid and at the same time it contains carbohydrates and vitamins. Alginates were extracted with a sequential extraction protocol from Sargassum vulgare. Structural characterization of alginate obtained from Sargassum vulgare was determined by FT-IR spectrum, phase structure by XRD diffractometer, and surface morphology by SEM image. Within the scope of the study, alginate obtained from Sargassurn vulgare and herbal preservative 705 used in the field of cosmetics were compared. After pretreatment of Sargassurn vulgare with ethanol, alginate extraction was performed. Microorganisms of Pseudontonas aeruginosa, Staphylococcus aureus, Candida albicans, Escherichia soli, Aspergillus brasiliensis were used to examine the antimicrobial activity of the obtained alginate and showed that the contamination risk was tolerable for all microorganisms examined on the seventh day of incubation. Alginate obtained from Sargassum vulgare was found to be more effective than herbal preservative 705 at 0 hours. Pseudomonas aeruginosa, Staphylococcus aureus are microorganisms with the highest effect in the 0 hour. Since alginate obtained from Sargassurn vulgare is more effective on microorganisms in a shorter time than herbal preservative 705, it is predicted that it can be a product that can be used in the field of cosmetics.

The outbreak of COVID-19 caused by SARS-CoV-2 has promptly spread worldwide. This study aimed to predict mature miRNA sequences in the SARS-CoV-2 genome, their effects on protein-protein interactions in the affected cells, and gene-drug relationships to detect possible drug candidates.
Methods: Viral hairpin structure prediction, classification of hairpins, mutational examination of precursor miRNA candidate sequences, Minimum Free Energy (MFE) and regional entropy analysis, mature miRNA sequences, target gene prediction, gene ontology enrichment, and Protein-Protein Interaction (PPI) analysis, and gene-drug interactions were performed.
Results: A total of 62 candidate hairpins were detected by VMir analysis. Three hairpin structures were classified as true precursor miRNAs by miRBoost. Five different mutations were detected in precursor miRNA sequences in 100 SARS-CoV-2 viral genomes. Mutations slightly elevated MFE values and entropy in precursor miRNAs. Gene ontology terms associated with fibrotic pathways and immune system were found to be enriched in PANTHER, KEGG and Wiki pathway analysis. PPI analysis showed a network between 60 genes. CytoHubba analysis showed SMAD1 as a hub gene in the network. The targets of the predicted miRNAs, FAM214A, PPM1E, NUFIP2 and FAT4, were downregulated in SARS-CoV-2 infected A549 cells.
Conclusion: miRNAs in the SARS-CoV-2 virus genome may contribute to the emergence of the Covid-19 infection by activating pathways associated with fibrosis in the cells infected by the virus and modulating the innate immune system. The hub protein between these pathways may be the SMAD1, which has an effective role in TGF signal transduction.
